Effective Date: September 14, 2026

At Precision IV, LLC (“we,” “our,” or “us”), we provide specialized mobile IV and vascular line services directly to patients and healthcare facilities. We know your health and personal information is private, and we take protecting it seriously. This policy explains in plain terms what information we collect, why we need it, how it is used, and how we keep it safe.

1. Our Commitment to Your Health Privacy

Federal and state laws (such as HIPAA) protect medical information. When we provide IV services, we follow strict healthcare privacy laws to ensure your information stays confidential, secure, and accessible only to people involved in your care or authorized by law.

2. Information We Collect

To safely place an IV line and keep your doctors informed, we collect a few essential details:

  • Contact & Identification: Your name, birthdate, contact details, and facility room or bed number.
  • Medical Details: Hospital paperwork (like a facesheet), provider orders, allergies, and the reason you need an IV line.
  • Procedure Notes: Details about the procedure itself—such as where the IV was placed, catheter size, equipment lot numbers, and how well the IV worked.
  • Staff Involved: Names of your ordering doctor, bedside nurse, and our clinician who placed the line.
  • Website Data: Basic technical data (like browser type or IP address) when using our website or digital tools.

3. How We Use This Information

We only use your information for clear, everyday care and business reasons:

  • Direct Patient Care: Making sure we have the right patient, choosing the right vein and catheter, and safely placing your IV.
  • Procedure Records: Keeping an accurate record of what supplies were used and how the procedure went.
  • Care Coordination: Updating your bedside nurse and doctors so they know your IV is in place and ready to use.
  • Billing & Facility Accounts: Ensuring our service is matched to the correct visit so facility accounts and invoices are accurate.

4. Sharing Your Information

We never sell, rent, or trade your personal information. We only share it with:

  • Your Care Team: The doctors, nurses, and hospital staff taking care of you so they have an up-to-date chart.
  • Secure Support Services: Highly secure technology platforms (such as encrypted cloud storage and secure form software) that help us run our documentation and operations safely.

5. How We Keep Your Data Safe

  • Digital Security: All computerized records are scrambled using modern encryption both when sent and when stored.
  • Strict Access: Only our trained clinical staff and authorized administrators can view your files.
  • Retention: We keep medical and service notes only as long as necessary to support your care and fulfill standard healthcare record-keeping needs.

6. Your Rights

You have clear rights regarding your personal information, including:

  • Asking to see or get a copy of your records.
  • Asking to fix mistakes in your personal details.
  • Asking who we have shared your information with.
  • Submitting a complaint to us or to the U.S. Department of Health and Human Services (HHS) if you ever feel your privacy was mishandled.
